With only the
fewest exceptions - full payment is expected when the
appointment is scheduled unless financial arrangements have been
made. We accept cash, personal check, and Visa, MasterCard, Discover,
and American Express for payment in full.

Specifics for
payment arrangements are detailed in your treatment
plan. Referral to private dental financing can be
made if needed. Patients needing financial
assistance can discuss the various options available
with our Financial Coordinator. |

Patients covered
by insurance are encouraged to provide complete and accurate
information about their dental insurance when they complete our
Patient Information form. Failure to complete the section
regarding insurance may result in the delay of your treatment.
We will ask for: name of head of household, their social
security number, their date of
birth, and for the name, address &
phone of their insurance company.
